If the visit is for your child or adult dependent, you'll both need to be present to see the clinician.
If you select a prescription to be shipped to your home, you must provide a residential address from within the state of service in order to receive the prescription (e.g., if you request a ship-to-home prescription through the MA program, you must provide an MA address to receive this prescription).
